If youre a motivated and patient-focused Clinical Pharmacist looking to make a genuine impact within general practice, this role offers the perfect blend of clinical autonomy, collaboration, and purpose. What youll do As a key member of the multidisciplinary team, youll: Provide high-quality pharmaceutical care and expert medicines advice to patients. Conduct medication reviews (including polypharmacy and SMRs) both face-to-face and remotely. Support GPs, nurses, and practice staff in resolving medication-related queries. Lead on medicines optimisation, quality improvement, and safe prescribing. Contribute to service development, audits, and QOF/local incentive workstreams. Play an active role in improving patient outcomes and access to healthcare. Youll work alongside an established network of pharmacists and pharmacy technicians, with the support of experienced senior pharmacists and practice colleagues.
