    Job summary An exciting opportunity has arisen for a support worker/trainee to join the assertive outreach and recovery team which supports patients in the community with their mental health recovery. The role involves working with people in their homes, the community clinics and also providing in reach to patients who are admitted to hospital to support their discharge home. The post holder will work as part of the multi-disciplinary team delivering high quality patient focused care in a community setting. The role will involve improving the lives of people with mental health issues and the impact of their condition through the delivery of excellent services to promote recovery and wellbeing. The role will be based at Lanchester Road Hospital but will also work into Teesside Community Rehabilitation Services at Lustrum Vale, Stockton, to provide supervision for Peer Practitioners based there. Peer support is when people with shared experiences connect to build safe, trusting and non-judgemental relationships where we learn and grow together.
