    Are you passionate about supporting and transforming the lives of people who are experiencing homelessness in Hammersmith & Fulham?  We are looking for a Project Worker to join our Hammersmith & Fulham Supported Accommodation Pathway. These services support adults with a range of needs who are preparing for more independent living.You will work across both Talgarth Road and Shepherds House as part of the wider H&F pathway, which is a large multi‑site service with a staff team of 58 supporting 129 clients. The pathway includes a mix of 24‑hour staffed complex needs services, semi‑independent step‑down accommodation, gender‑specific provision, and specialist support including domestic abuse services and employment, training and education support.In the role of Project Worker, you will support clients on their journey toward increased independence. You will manage a caseload of residents, helping them develop the skills and confidence required to move on to appropriate longer‑term or independent accommodation. The service focuses on supporting residents around their strengths and aspiration's for the future.This is an exciting opportunity to join our new Assessment Hub team at Apple Tree Cottage, providing rapid assessment and support into appropriate accommodation to clients over a 28-day period, ensuring those at risk of rough sleeping and homelessness can be directed to services suitable to their immediate, medium and longer term needs. The role of a Duty Worker is to jointly manage the safe day to day running of the project with other members of the team and to provide support and advice to clients. The first point of contact at Reception or over the phone for clients and visitorsResponsible for coordinating the Health and Safety within the service with the support of the Service Manager. Expected to regularly participate in team meetings and other ad hoc project groups as agreed with the management and team of the project.Please note, due to the needs of our services, there is a requirement to work a shift rota, so flexibility is required.
