    Travel to various locations will be required for this role. Some work outside normal hours may be required as per the needs of the service.

This post is part of a NHS Vanguard pilot, Work Well, the project will build on the success of our single access point and our Facebook group Get Rochdale Working, supporting employed residents who have a health condition or disability or those recently unemployed, by offering advice about the most appropriate options and understanding the barriers people face. This role involves coaching and motivating residents to succeed in achieving their goals or accessing alternative provision if required. This role will involve having a caseload of residents, supporting them through a personalised action plan in order to achieve their potential.The successful applicant will be part of the Get Rochdale Working team, utilise social media to liaise with residents, attend events and have a good understanding of the work and skills offer across the Rochdale Borough so that they can guide residents to the right support at the right time. The team also work closely and receive referrals from a wide range of partners including; council staff, police, social housing providers, NHS, public health, social care and drug and alcohol professionals to help people to change their behaviours and improve their lives.
