We serve a stable patient population of around 8,450 patients from spacious, purpose-built premises. We pride ourselves on being a genuinely friendly and close-knit team. The practice is led by two partners and supported by five salaried GPs, two practice nurses, and CTAC nursing staff. Our multidisciplinary team includes an on-site Pharmacist and Physiotherapist, all working closely within the practice to support patient care. A strong sense of teamwork and wellbeing is central to our culture. We value taking time to connect with one another and the whole team comes together daily for a shared tea/coffee break a small but important part of maintaining a supportive and collegiate working environment. Our experienced and approachable administration team provide excellent support, allowing clinicians to focus on patient care. You will have a key role in ensuring that patients have a confirmed diagnosis as early as possible, supporting those patients in all stages of the pathway by being a vital point of contact. Excellent IT skills and working knowledge of Excel and database management is essential. This role has been created alongside a new Occupational Therapist role, strengthening our multidisciplinary team and supporting our ambition to deliver high‑quality, proactive care to older adults living with frailty in the community.About the ServiceFrailty at Home is a forward‑thinking service that works to identify deterioration early, support rehabilitation at home, and provide alternatives to hospital admission.
