The postholder will be responsible for providing oversight and management of the Surgical Pre-Operative Assessment (POA) patient pathway across Trust sites, ensuring a high quality patient experience and supporting service development to maintain this. Detailed job description and main responsibilities Provide guidance and operate as the person of expertise for the service and patient pathways. Ensure effective processes and procedures are in place to monitor and track performance to meet the service and Trust targets, participating in and/or provide data information for audits. Report on POA waiting lists including follow up activity, ensuring appropriate ensuring escalation regarding delays or backlogs. Provide advice and guidance where necessary to the Surgical Admission booking teams using POA service knowledge to support RTT compliance. Monitor POA capacity and work with nursing and operational teams to identify solutions where capacity pressures arise.  To identify and work with the nursing and operational team to implement service improvements. To investigate patient complaints as directed by the operational team in line with Trust policy. Support nursing teams with referral and triage related administrative processes to ensure timely progression through POA. Supervise, support and organise the day to day running of the service, identifying resource needs, distributing workloads and ensuring staff levels are adequate to meet demand and priorities. Monitor quality of patient care delivered, ensuring high standards are maintained at all times and patient enquiries are dealt with efficiently, professionally and appropriately. The Royal Borough of Greenwich Council and the South East London Integrated Care Board have come together to create an integrated approach to commissioning services within Greenwich, this postholder has a key role in implementing this new approach. Key Result Areas & Performance: This role will work collaboratively with relevant teams in the acute and community services to identify opportunities for improvements in flow pathways. The opportunities may proactively prevent avoidable admissions (those with no clinical requirement), or facilitate people being discharged from acute services/community hospitals into community and/or social care provision. The purpose of the role is to ensure that only those people requiring acute care are admitted to hospital and these are then discharged to the appropriate destination following the completion of their acute treatment. The role will maintain oversight of the patients on the TOCC list ensuring all actions not completed within agreed timeframes are escalated through daily touchpoints with community health and social care partners following an escalation cascade when necessary. This role will work closely with the TOCC list co-ordinator and TOC hub manager to facilitate flow and timely updates as required Main duties of the job Main duties To monitor and ensure a responsive service (across the range of support services for discharge) for complex discharge of patients from acute trusts. To ensure that patients receive safe and effective discharge care from the range of providers (community, social care, continuing healthcare) to meet their individual care needs in a timely manner. To identify within the current systems and processes “blockages” that are preventing effective discharge and to provide recommendations on solutions to the ToCH Lead and alerting senior leaders across the system as appropriate.  To implement with partners and providers any recommendations agreed and ensure timely service change. The Imaging Department at University Hospital Lewisham (UHL) is looking for a full-time (37.5hr) pathway manager to lead a small bookings team with responsibility for ensuring the smooth running of the patient's diagnostic pathway. To provide autonomy in specialised diagnostic services. 23)Immediate Life Support trained, with regular up stress echo studies. 24)24 hour ECG tape preparation and analysis as when required 25)To attend Mandatory sessions(, internal or external) and courses related to development Work force 1)To manage own work, staff, departmental services and patient care and develop autonomous skills, liaising with clinicians and directorate managers if required. 2)To attend any relevant study days, in-house lectures, training afternoons and quality group meetings. 3)To participate in departmental audit. 4)To offer advice and guidance to patients, carers and other health professionals on cardiology procedures of patients conditions. 5)To inform and promote guidelines and departmental policies to other healthcare professionals 6)To obtain informed consent from patients prior to undertaking procedures as appropriate to individual circumstances 7)To ensure accurate recording on Current EPR systems and examination details. 8)To promote good relations with other directorates and hospitals 9)To provide independent ECG, Exercise ECG, 24 hour analysis diagnostic results and updated guidelines for all inpatients, outpatients, open access services for the community and local and specific population. 10)To successfully achieve and uphold up to date certificates for skills and competencies, maintaining numbers as part of quality controls Financial Monitor, order and check supplies of goods and/or services Partnerships ·Communications and Relationships ·To Instruct patients and to get consent.
