You will support children and young people with complex health needs to access their education. The Special Schools Nursing network provides support to children and young people with complex health needs. The nurses assess and identify any care needs that can be delegated to school staff by ensuring that the skill is risk assessed and school staff are trained, supervised and competent to deliver the care appropriate for the student within designated Special Schools. This is a term-time only role that would suit an enthusiastic Special School Nurse ready to join our fantastic nursing team at Nexus School, Tonbridge, Kent. Main duties of the job We work with families and partnership agencies to support school staff and make sure they are appropriately trained and competent for students to access school safely. We want to make sure the school staff understand the child's condition and the potential impact it has on the student and their families. This role will support the staff who provide one to one care to some of the students with more complex respiratory health care needs. Training in the appropriate skills will be provided plus there is access to a wide training programme at KCHFT. If you have experience of working with children with complex health needs within a health, educational or social care setting, would like to use and build upon existing knowledge and skills, enjoy training others and working within a flexible and friendly team then this could be the perfect role for you. Urgent Care Services encompass a variety of offerings under one umbrella, with staff collaborating through an integrated approach across different services. The teams focus on preventing unnecessary hospital admissions and supporting timely discharges. We must respond swiftly to urgent needs, requiring a flexible workforce capable of delivering medical, nursing, and therapy interventions. Managing complex cases, the service also provides guidance toward long-term support and anticipatory care planning for patients with chronic conditions. Out-of-hours unscheduled care is offered for patients who may be at the end of life or facing urgent healthcare needs, which can include treating acutely unwell individuals to avoid hospital admission and supporting hospital discharges. The Hospital at Home Service is a nurse-led model that enables patients to leave the hospital once clinically stable and complete their acute care at home. This service is a collaboration between MTW and KCHFT. The Virtual Ward model, in partnership with the acute trust, delivers hospital-level treatment at home. Unlike Hospital at Home, it is specialist-led, with patients virtually admitted to the acute trust and monitored through the Virtual Ward Hub while being treated at home. The service provides nursing care within patients’ homes across the West Kent area, and a valid driver’s license is essential for this role. Main duties of the job Shifts run from 08:00 to 21:00, seven days a week, with an average of 3-4 clinical shifts per week. The team gathers at the base at the start and end of each shift, with additional check-ins during the day.     You will work alongside your allied health professional colleagues to deliver our recently established symptom management programme in a 1-1 and group setting. Your role will also include monitoring and assisting with equipment provision.
