You will use your knowledge, experience, and caring nature to make a positive difference in the lives of women and families who are underrepresented and affected by domestic violence and abuse. You will work primarily with Global Majority communities, including South Asian women and their families, providing one-to-one and group support. You will help women explore their options and rights for themselves and their children, taking a whole-family approach and providing wraparound support to access specialist services where needed. Key Responsibilities: Deliver trauma-informed one-to-one and group support Empower women and families to understand their rights and options Provide holistic, wraparound support and referral to specialist services Maintain accurate records and uphold confidentiality Work collaboratively with partner agencies Preferably have proficiency in Bengali/Sylheti to support communication with clients from relevant communities. If so, we at Age UK Leeds are recruiting for an Hospital to Home Support Worker to work as part of our Hospital to Home. Service: Hospital to Home Support Worker  Salary: Scale 5 £26,459 pro rata Hours: Part Time: 56 hours over the course of a fortnight. This averages at 28 hours per week and requires working alternate weekends. Current service hours are between 08:30am and 7pm but these may alter based on the needs of the service and funding. About the role In this role you will work as part of the Hospital to Home Team, who provide transportation to facilitate hospital discharge and who carry out support tasks within people’s homes to prevent further hospital admissions. The team, also, assess individuals’ wider support needs and effectively signpost and refer onto other organisations to ensure longer term support is put in place. About the Hospital to Home Service Our service prides itself on its “Home First” and patient-centred approach and supports those over 50, who have suffered a period of illness that has affected their independence following a hospital admission or visit to A&E. We work closely with professionals from across the Health & Social Care sector and a range of other agencies to encourage clients to access services that will improve their health and wellbeing and retain their independence. This may involve activities such as a Pre-Discharge Home Check to ensure that the patient’s home is secure, warm and ready for their arrival and/or identifying potential support needs based upon each individual’s needs, such as hand rails/trip risks etc. About you You must have good communication and organisational skills and the ability to assess and respond to the needs of older people. Problem solving and using your initiative, whether lone-working or as part of a team, to provide support, you will work from our base at St James’ University Hospital and out in the community. This is a hugely rewarding role delivering many different aspects of emotional and practical support to a wide range of patients. The consortia have GIPSIL as the lead provider alongside Inspire North & Turning Lives Around. OWL provides a wide range of interventions to prevent homelessness, four types of supported housing and pre and post-tenancy support schemes for young people & young people leaving care including unaccompanied asylum seeker children. Young people can be single, young families or couples. The main responsibilities of the post include: Using strength-based, restorative and trauma informed practice putting young people at the centre of their support. Work independently, often lone working for long periods, providing support in a 24/7 living environment for young people making the transition to independent living. To facilitate support in order to ensure young people develop domestic skills, parenting skills, life skills, social skills and behaviour management skills, where appropriate, to sustain a tenancy and home. Alongside the young person, professionals and close family members develop and implement a support plan and the risk assessment; Play a critical role in ensuring there is smooth handover to the night support staff.
