The service offers 30 units of accommodation across five sites, together with a floating support service that helps residents make a smooth transition into independent living and provides ongoing support to individuals within the community who require additional assistance.This role will operate across different levels of supported housing, including a flexible threshold unit that adopts a harm reduction approach and delivers community-based support tailored to individual needs.As a Senior Practitioner, you will play a key role in coordinating and overseeing casework, care management, and specific service projects.
