Holds delegated responsibility for management of the ward on a shift basis. Commencing discharge planning at the point of admission in collaboration with families/carers and social care where appropriate. Undertaking/ overseeing appropriate assessments and consulting with social care staff, funding panels and future care placements. Highlighting any difficulties with placement of patients to senior management team and providing Clinically Ready for Discharge (CRFD) but delayed reports as required. Ensuring close working relationships with all professionals across the pathway both internally and externally. Clearly identify if the reason for admission was due to placement breakdown or lack of suitable placement in the community Ensure discharge planning begins on admission with system involvement including Care Coordinator/Social Worker and Commissioner, clearly identified in the 12-step discharge plan. Provides a discharge update for each patient at their weekly MDT clinical review, following up outstanding actions and liaising with system partners in preparation.  Any barriers to discharge to be noted at the MDT clinical review meeting and escalated to line manager and Head of Programme and subsequently reviewed on a weekly basis.  Ensures all relevant system partners are made aware when a patient is identified as Clinically Ready for Discharge Be fully integrated into the ATU teams across the system and be flexible to team pressures.
