If you believe that there are ways to drastically improve the way primary care is delivered for both patients and staff alike and you want to be a part of this; then we would love to meet you and tell you more about our plans. Job Purpose The Prescribing Nurse Practitioner will play a key role within our multidisciplinary team, delivering clinical assessment, diagnosis, treatment, and prescribing for patients in a primary care setting. This role includes home visits, support with QOF chronic disease reviews, undertaking clinical same day assessments, and ongoing continuity of care. The post holder will work autonomously within their scope of practice, helping to increase appointment capacity and improve access for patients. Key Responsibilities Undertake triage and clinical assessment of patients, including same day, and routine patients.  Independently manage a wide range of conditions, providing evidence-based treatment and prescribing in line with local guidelines.  Conduct home visits, providing high-quality care outside the practice setting which may include frailty checks, wound care, injections and medication reviews.Deliver QOF aligned chronic disease management. Provide medication reviews, reconciliation, and support safe medicines optimisation. Offer lifestyle advice and signpost patients to community or social prescribing services. Work closely with GPs, nurses, pharmacists, and the wider MDT. Attend clinical meetings, safeguarding reviews, and case discussions. Provide clear, compassionate communication to patients and carers, including handling complex or sensitive information. Adhere to confidentiality, safeguarding, and information governance standards. Lead and oversee all infection-prevention and control processesParticipate in quality improvement and audits where appropriate. Maintain accurate clinical records and complete all required documentation.

The ACP plays a key role in reducing unnecessary GP referrals, ensuring patients receive timely and effective care, and developing long-term relationships with patients to support optimal health outcomes. As an ACP, you will make independent clinical decisions, interpret test results, prescribe medications, and refer patients to specialists when appropriate. The post holder may be required to work autonomously for parts of the week when no GP is onsite, with remote clinical support available as needed. Key Responsibilities Assess, diagnose, and manage patients with acute and chronic conditions. 
     Order, interpret, and act on investigations (blood tests, imaging, ECGs, spirometry). 
     Prescribe and review medications independently. 
     Perform minor procedures (wound care, sutures/staples, joint injections if trained). 
     Develop and implement individualised care and treatment plans. 
     Manage long-term condition clinics (diabetes, hypertension, COPD). 
     Provide health promotion, disease prevention, and lifestyle advice. 
     Triage patients and decide on escalation or referral to GPs or specialists. 
     Provide patient education, self-management support, and follow-up care. 
     Contribute to service planning and improvement initiatives. 
     Support clinical audits and quality improvement projects. 
     Support practice in implementing evidence-based guidelines and protocols (NICE, local policies). 
     Provide clinical leadership to junior staff, nurses, or other allied health professionals.

     Participate in revalidation, CPD, and professional development activities.

     Essential Criteria Registered Nurse, Paramedic, or relevant healthcare professional with NMC/HCPC registration. 
     Meets the standards for Advanced Clinical Practice (ACP) at advanced level. 
     Qualified Independent Prescriber. 
     Meets professional revalidation requirements. 
     Master’s degree in Advanced Clinical Practice. 
     Experience in prescribing and conducting medication reviews.
