    Our specialist Supported Living and Outreach services in Beaconsfield provides support for people with Learning Disabilities. We understand the importance of providing person-centred support that enables independence and meets each individual's needs.As a Support Worker, you will be involved in the day to day support and routine of the people within the service including:Promoting and delivering a person-centred support culture across the serviceSupporting people with all aspects of their daily living as per their support plan, this will include personal care and taking their medicationSupporting and coaching people to learn independent living skills like cooking and laundry tasksSupporting people to access the community for social, educational or employment opportunitiesSafeguarding service users’ rights and dignityTaking an active role in peoples support plans and risk assessment’sProvide effective communication within the team and attending regular team & service user meetingsTo liaise professionally with family members and other professionals.A positive attitude & passionate about supporting adults with learning disabilitiesCan demonstrate a practical understanding of delivering support with dignity, privacy and respect
