To support the reablement of service users, including short-term as well as long-term residents. To act as shift leader and take charge of the Care Home as necessary. To act as key worker for a number of service users, who may be resident in the Care Home or who may use day care facilities. This may include continuing to work with these service users if they are temporarily moved to other facilities eg for recuperation or reablement.Following appropriate training, to act as shift leader as and when required. This involves having temporary charge of the Care Home, including being responsible all staffing matters during that period. To support reablement plans for service users (maximising individuals’ potential). To administer medication as prescribed. To organise and support social activities and outings to enhance the quality of life for service users. To enable service users to maintain contacts in potentially isolating situations.
